Pearl Home Care can provideshort and long-term supports to strengthen the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community.

Level One and Two Support Coordination
Pearl Home Care understands and acknowledges that it can be daunting engaging with providers to deliver the supports and services you require to maintain your independence and quality of life. Our team are trained to assist you to ease this burden.
In consultation with you and/or your substitute decision maker, Pearl Home Care will identify the supports and services most important to you. We will support you by contacting suitable providers to enquire about their supports and the fees associated. This information will be presented to you, for you to make the decision around your supports. It is imperative that you feel confident and empowered to make your own decisions around your care.
Overtime with our ongoing support and training, you will have the capacity and confidence to engage with providers and make decisions around your supports and services.
