What Should I Bring To My Meeting With Pearl Home Care?
If you are looking to switch to Pearl Home Care, all you need to bring is:
- A copy of your current Home Care / Service Agreement
- Documentation showing your funding approval
- Personal identification documents
- Any income assessments
You may have an advocate or other representative assist you, so you can communicate your needs and goals, and we can ensure our services are the best fit for you.
Your Pearl Home Care team will then assist you to make transition from your current provider as seamless as possible, including:
- Checking your Home Care / Service Agreement for any notice periods or exit fees (these are not applicable to aged care)
- Assisting you to inform your current provider in writing of the cessation date of your Agreement.
- Assisting you to contact My Aged Care (for aged care clients) or your Support Coordinator or Plan Manager (for NDIS Participants) to inform them of your intention to move providers
- Activating services in relevant government portals or with coordinators/managers
- Agreeing a Care Plan, support schedule, new Agreement and care team that achieves your goals
- Delivering services immediately following the last day of services from your previous provider, ensure no loss of services and seamless transition
